The “Eshkiniigjik Naandwechigegamig - A Place for Healing Our Youth" – “Aabiish Gaa Binjibaaying – Where Did We Come From?” is an Indigenous Youth Agency dedicated to providing programming and services to support Indigenous youth in North America. Our mission is to help them become responsible, participating citizens while leading their Indigenous communities.

Position Summary

Under the direct supervision of the Case Manager Supervisor the Case Manager will be responsible for providing coordinated care and ensuring a culturally safe, trauma-informed approach to Indigenous youth aged 12-29. This role will have a high concentration of housing support, assisting youth in navigating housing services and ensuring that their living conditions contribute positively to their well-being. The Case Manager will work directly with youth, ensuring their involvement in activities, promoting quality care, and maintaining strong collaborations with other service providers.

Key Responsibilities

  • Provide coordinated care based on the individual needs of youth aged 12-29.
  • Ensure culturally safe and trauma-informed approaches to service delivery.
  • Help youth navigate housing services, ensure stable, safe living arrangements, and support youth in transitioning to independent living.
  • Ensure all youth accessing ENAGB register for the program.
  • Complete the Ontario Common Assessment of Needs (OCAN) for all youth registering for coordinated care.
  • Enter case notes into the data-based system.
  • Support proposal applications to future funders.
  • Develop collaborations with other service-providing agencies.
  • Attend Youth Council Meetings (when required).
  • Engage directly with youth to encourage participation in activities.
  • Collaborate with the team to create a positive, supportive atmosphere.
  • Ensure confidentiality of privileged information.
  • Adhere to ENAGB Youth Program policies and procedures.
  • Participate in ongoing professional development activities to enhance skills and knowledge.

Qualifications 

  • Bachelor in Social Work, Child & Youth Worker, or relevant experience.
  • Minimum of 2 years of experience working with youth (12-29 years).
  • Extensive knowledge of the history, culture, and traditions of Indigenous peoples.
  • Fluency in an Indigenous language is an asset.
  • Strong experience and knowledge in providing housing support and navigating housing services.
  • Ability to work evenings and weekends, as required.
  • Adequate energy and ability to meet the physical demands of the job.
  • Valid Standard First Aid and CPR certification.
  • Cleared Criminal Reference Check (CPIC) and Vulnerable Sector Screening (VSC) is mandatory.
  • Positive attitude, self-motivation, and the ability to work effectively within a team.

Compensation

  • Salary: $43.87/hr
  • Hours: 37.5 hours/week

Contract Duration

  • 1 year with possibility of extension
