Schedule: Full-time, 8:00 am – 4:00 pm, five days per week including Saturday and Sunday.

Reporting to: Director of Clinical Programs

Salary: $54,409.10

Since 1933, Haven Toronto has been a drop-in centre where elderly homeless, marginally housed and socially isolated men can feel safe, where they can form and cultivate friendships, and become part of a community. Our vision is a safe and just community in which elder homeless men can live a quality and dignified life.

Haven Toronto is actively seeking a Case Manager to work in our Life Enrichment Program. This is an exciting and unique professional opportunity to provide a mix of drop-in, social recreation and housing-focused case management services to older men living with homelessness or precarious housing.

Summary: Reporting to the Director of Clinical Programs, the Case Manager will work closely with clients, 
staff, and volunteers to enhance the quality of life and overall well-being of our community members.

Key Responsibilities:
• Assist in the implementation of social, recreational, and wellness programs (e.g., arts, fitness, 
games, cultural outings, group activities) that foster community engagement and connection.
• Collaborate with team members to assess client needs and develop tailored support plans.
• Provide direct support to clients, including individual and group counselling, and crisis intervention.
• Facilitate workshops and group sessions on topics related to mental health, addiction, and housing.
• Coordinate with external agencies and service providers to ensure clients have access to the 
resources they need.
• Assist Support Care Workers and other colleagues in the operation of the organization as required.

Qualifications:
• Degree in social work, psychology, counseling, or a related field.
• Experience in facilitating social/recreational programs, ideally in a community or social services 
setting.
• Strong understanding of trauma-informed care and harm reduction principles.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Commitment to working weekends and ability to maintain punctuality in an 8 am – 4 pm schedule.
