Job Summary:
As a Case Manager, Psychogeriatric, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. This is a permanent full-time role that will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
The Path Home – Pine Villa is a short- term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long-term living arrangement. Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment. The Path Home – Pine Villa, provides client centered and goal-oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing.
The Path Home – Pine Villa is a unique partnership between LOFT Community Services, SPRINT Senior Care and Sunnybrook Life Sciences Centre. This partnership reflects the transformation of health care in Ontario which brings the expertise and some of the resources of a hospital setting to a community based transitional housing program. Some of these resources include on site clinical practitioners including an Occupational Therapist, Physiotherapist, Nurse Practitioner and Advanced Practice Nurse.
