Job Summary:
The Case Manager on the M-DOT Hotel Team provides direct service to homeless individuals living with mental health, health, and addictions issues. The position provides both on site and community-based assessment and case management services to individuals living in various physical distancing shelter hotel programs through the city of Toronto. Case Managers work as part of a team that includes Team Leaders, Peer Support Workers, and Housing Support Workers, to meet clients’ housing and other social and health needs. The program works closely with nurses and physicians from Inner City Health Associates, to assess, plan, treat and support clients. The position will involve driving and a valid Ontario driver’s license and insurance are required. Access to a car is a requirement.
M-DOT is a specialized team of providers from LOFT, Inner City Health Associates (ICHA) and the Centre for Addictions and Mental Health (CAMH), that delivers services to the most vulnerable individuals living and sleeping on the streets of Toronto. The Team is comprised of 1 Team Lead (LOFT), 5 case managers (LOFT), 1 peer support worker (LOFT), 2 psychiatrists (ICHA), 1 nurse (ICHA) and 1 Concurrent Disorders Social Worker (CAMH). M-DOT provides clinical hands-on support to those with complex healthcare needs, who are experiencing chronic homelessness. M-DOT provides outreach and case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy. Services are designed to be short-term, focused on comprehensive assessment, stabilization and linking to long term resources. M-DOT provides services to individuals often identified as ‘complex’, who have not responded positively to other programs or attempts to engage. The aim of M-DOT is to provide a low barrier transitional service, that meets individuals where they are at and assists in stabilizing their health, mental health and substance use. M-DOT works from a housing first and harm reduction approach. The goal is to meet immediate needs, reduce barriers and assist individuals in getting connected to housing and follow up support that meets the level of need required for each individual to live successfully in the community.
