Job Summary:
As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and selfrespect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
Back to Home - East Toronto is a 70-unit program that includes 10 transitional short-term units, 2 respite, and 58 permanent housing. The program offers: Affordable supportive housing, 24/7 PSW care, 24/7 nursing support, care coordination, crisis intervention, system navigation and client advocacy, Behavioural Supports, Life enrichment services, primary care, psychiatry and pharmacy services.

Work Environment:
Staff will be required to work in clients’ rooms and may therefore be exposed to secondhand smoke.
May also be required to be on-call as emergency contact for after hours.
Frequent travel within the Central LHIN will also be required.
Occasional evening and weekend hours may be required.
