Job Summary:
As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues in a Crosslinks residence or on their own to various community services. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
Multidisciplinary Access to Care and Housing- MATCH is a specialized team of providers from LOFT and Inner City Health Associates (ICHA), that delivers services to the most vulnerable individuals to assist in accessing and maintaining housing and connecting to health and other supports in the community. The MATCH team consists of 11 case managers, 2 registered nurses (shared), 2 peer support workers and 2 part time psychiatrists (ICHA). MATCH provides clinical hands-on support to those with complex healthcare needs through case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy. MATCH works with individuals who have previously and/ or are currently experiencing homelessness, whose health and needs are stabilized while working with the CATCH and M-DOT teams. MATCH provides long term multi-disciplinary wrap around and case management and health services in their (often new) communities. The aim of MATCH is to improve transitions of care for individuals and works with them to identify and meet their goals toward recovery, while gaining stability and building community
