Job Summary:
As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and selfrespect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
North Toronto ICM Team provides assistance to adults living in North York and North Toronto who are experiencing mental health issues that seriously impact their life. The program provides open ended and short-term case management and peer support services. The program provides assessment, consultation, access to treatment, service planning and advocacy for individuals referred through The Access Point. Services are centered around each individuals needs and focused on comprehensive assessment, stabilization and linking to resources in the community. The aim of the program is to provide a low barrier service, that meets individuals where they are at and assists in stabilizing their health, mental health and substance use. The program strives to assist individuals to make choices and changes in their lives that promote recovery.
