Location: Onsite (CarePoint Health – 2695 North Sheridan Way, Suite 120, Mississauga, ON), but may also be required to work at other CarePoint Health sites within Mississauga with work from home options.
Preferred start date: December/January
Salary: $46,487-67-$54,691.38 and a comprehensive benefits package (including HOOPP pension)
Starting salary adjusted to commiserate with experience.

About CarePoint Connects
CarePoint Connects is a community-based initiative, led by CarePoint Health in collaboration with the Mississauga Ontario Health Team (MOHT), to help residents without a family doctor connect to primary care providers. The program strengthens coordination across local healthcare partners and ensures that every individual in Mississauga has access to timely, equitable, and continuous care.
What is this role?
Reporting to the Clinical Manager, the Client Coordinator serves as the primary point of contact for CarePoint Connects. This role is essential in supporting patients throughout their care journey—from initial intake to successful connection with a primary care provider—ensuring that no patient falls through the cracks.
The role requires excellent communication, organization, and problem-solving skills to coordinate across internal teams and community partners while maintaining accurate records and supporting a positive patient experience. The ideal candidate has experience engaging directly with patients, coordinating services, and working collaboratively across healthcare and community sectors.

Key Duties and Responsibilities
- Serve as the main point of contact for patient inquiries by phone and email.
- Conduct patient intake and screening, ensuring all referral data is complete and accurately documented.
- Track and monitor patient progress through all stages of the CarePoint Connects process to ensure timely follow-up and continuity of care.
- Maintain accurate and up-to-date patient information across multiple digital systems and databases.
- Coordinate patient matches with family physicians based on program criteria and mutual fit.
- Liaise with family physician practices and the Physician Engagement Team to confirm capacity, service offerings, and intake processes.
- Collaborate with the Nursing Team to connect patients with active healthcare needs to appropriate clinical and transitional supports.
- Work with primary care, Health Care Connect, community, and social service partners to address barriers and improve access to care.
- Support and participate in community outreach, engagement, and promotional activities for CarePoint Connects.
- Contribute to program evaluation, continuous improvement, and the implementation of new practices and processes.
- Stay informed about primary care initiatives and system navigation resources across Mississauga.
- Participate in training and ongoing professional development.
Key Qualifications and Skills
- University degree and/or Community College Diploma in social services or related field;
- 2-3 years of experience in mental health/social services role preferably in an intake or service navigation role
- Exceptional interpersonal, verbal, and written communication skills.
- Strong problem-solving, decision-making, organizational, and time management abilities, with the capacity to manage multiple priorities in a fast-paced environment.
- Proven ability to build and maintain effective working relationships with inter-professional teams, community partners, and external agencies.
- Demonstrated commitment to providing genuine, respectful, and inclusive care to diverse and vulnerable populations, grounded in principles of equity, diversity, and inclusion.
- Knowledge of community healthcare and social service resources within Mississauga is an asset.
- Comfortable conducting community outreach, delivering presentations, and engaging with groups to raise program awareness.
- Strong attention to detail in collecting, documenting, and reporting client and program information.
- Proficient in or able to quickly learn a variety of digital tools and platforms. Previous experience with an EMR is an asset.
- Fluency in a second language is considered a strong asset.
- Self-directed, adaptable, and committed to continuous learning and improvement.
- Demonstrated alignment with CarePoint Health’s values and a commitment to fostering a positive, innovative, and collaborative work environment.
- Satisfactory and current Criminal Background and Vulnerable Sector Screening required.
- Vaccinations (COVID-19 and others) are required unless exempt on medical grounds under the Ontario Human Rights Code.
