Patient Ombudsman helps resolve complaints from patients, residents and caregivers about their experiences in Ontario’s public hospitals, long-term care homes, home care, and community surgical and diagnostic centres. Patient Ombudsman can also undertake investigations into experiences at these organizations, either from complex complaints or as an own motion investigation that looks into systemic or larger issues that are of high public interest.

We are seeking an Investigator with five or more years of investigation experience to join our office. This position focuses on fair, independent investigations into systemic health care issues, including interviewing, evidence analysis, report writing, and working with health care organizations to support resolution and accountability.

The role requires strong analytical, interviewing, and reporting skills, and is well‑suited to professionals with backgrounds in governance, compliance, regulatory oversight, or public‑sector investigations. It does not involve clinical decision‑making or law‑enforcement investigations.

Through our engagement with Indigenous patients, organizations and health care leaders, Patient Ombudsman’s work to resolve complaints and investigate systemic issues has already benefited from the sharing of Indigenous knowledge, lived experience, and culturally grounded approaches to fairness, accountability and resolution.
