We are currently recruiting for a Primary Health Care Nurse Practitioner

Schedule: Days, some evening and occasional weekends

FTE: 2.0

Location: Multi-site: This position will be located primarily at new clinic and/or drop-down clinic spaces based in East Mississauga. The position will also occasionally work out of CarePoint Health’s main clinic location at CarePoint Health – 2695 North Sheridan Way, Suite 120, Mississauga, ON

Salary: $109,021-$129,021 and a comprehensive benefits package (including HOOPP pension)

CPH is also the Network Lead Organization for the Ontario Structured Psychotherapy (OSP) Program in Brampton, Halton, Mississauga and South Etobicoke. OSP offers adults (18+) in Ontario free, short-term, evidence-based cognitive-behavioural therapy and related services for depression, anxiety, and anxiety-related conditions.

What is this role?

CarePoint Health is seeking a Primary Health Care Nurse Practitioner (NP). The NP will also work in collaboration with the affiliated family physicians and the interdisciplinary team to provide timely, effective and appropriate intervention for patients. The NP will also provide comprehensive primary care services for unattached patients within CPH’s geographic area. The NP will utilize his/her advanced knowledge and decision-making skills in assessment, diagnosis, and health care management, thus providing comprehensive health services to patients who include: the patient throughout his or her lifespan with a strong emphasis on health promotion and disease prevention. This expert practitioner’s role is to contribute to the quality of patient care through advanced practice, education, and consultation. The NP will hold a valid driver’s license and be willing to drive to other CPH sites within Mississauga and for home visits.

Responsibilities:

Reporting to the Clinical Manager your key responsibilities would include:

  • Overseeing the delivery of programs including, but not limited to: primary care for unattached patients, newcomer’s clinic, supportive care, women’s health, lung health, episodic care, chronic disease management and cancer screening clinic.
  • Conduct home visits for home-bound patients and frail seniors.
  • Work collaboratively in a care team to provide comprehensive care to patients while demonstrating a high degree of emotional quotient, empathy, and clinical expertise.
  • Care for patients with both episodic and chronic illnesses and assist in addressing both medical and psychosocial needs of patients including those with multiple morbidities.
  • Perform clinical assessments combined with sound clinical judgement/diagnostic and treatment skills, within full scope of practice as defined by the College of Nurses of Ontario CNO)
  • Demonstrate leadership skills by working effectively within an inter-professional and proactive team environment, exercising excellent communication and interpersonal skills with both medical and non-medical staff and with patients and their family members.
  • Able to delegate tasks to the nursing staff as appropriate.
  • Actively participate, seek and provide consultation with external health service providers in the patient’s circle-of-care.
  • Actively include community development and outreach and resource management to community partners and stakeholders
  • Contribute to activities related to the development, implementation and evaluation of medical directive, policies and procedures, best-practices, and protocols.
  • Contribute to the development of services and programs.
  • Participate in chart reviews and development/implementation of Quality Improvement Plans
  • Demonstrate active participation in the CNO Quality Assurance Program to ensure maintenance of continuing competence to practice.
  • Participate in the design, implementation, and evaluation of research studies that CarePoint Health may participate in, from time-to-time.
  • Perform other duties as assigned that are reasonable with the scope of practice.

Qualifications:

  • Completion of a recognized Primary Care Nurse Practitioner Program - (RN (EC))
  • Baccalaureate in Nursing – Masters preferred.
  • Current registration as a Registered Nurse in the Extended Class (RN (EC)) with the Ontario College of Nurses
  • 3-5 years demonstrated experience in a community and/or Primary Care Setting such as within Nurse-led clinics.
  • Demonstrated experience in chronic disease management.
  • Demonstrated ability to work effectively in a multi-disciplinary environment.
  • Demonstrated commitment to, and knowledge of, community-based health care.
  • Awareness of and sensitivity to the health issues of a diverse community and experience working with vulnerable populations
  • Excellent communication, interpersonal, organizational, and administrative skills that include computer literacy.
  • Experience with electronic medical records.
  • Experienced working with at-risk/vulnerable population
  • Be sensitive to patients’ issues and needs and have the capacity to address these issues.
  • Contribute to program development.
  • Ensure all communications reflect CPH standards and facilitate a positive patient, family, and provider experience.
  • Uphold CPH values and be an active contributor to a positive, innovative, collaborative, and cooperative work environment.
  • Good reading and writing communication skills in English, a second language would be an asset.
  • BLS certification required.
  • A satisfactory and current Criminal Background and Vulnerable Sector Screening and proof of immunization required.
  • Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
  • Acts as a professional and knowledgeable role model or mentor to colleagues and students; and
  • Maintain confidentiality in the workplace and within the community.
  • This position will be based out of multiple CPH sites and travelling will be required.
  • A valid Ontario Drivers’ License and own vehicle required
