Job Description:

  • Provides client–centered comprehensive primary and episodic health care, as outlined and appropriate within the Extended Class scope of practice.
  • Facilitates discussions with clients to determine existing and potential health concerns with a focus on related lifestyle, psychological and environmental risk factors and assessments, to stabilize and improve client’s health status.
  • Provides advanced nursing care and treatment; monitors ongoing therapy of patients; implements strategies to promote independent and healthy lifestyles.
  • Works within established care pathways, medical directives, guidelines and protocols
  • Orders appropriate investigations and interprets results of screening and diagnostic laboratory tests; determines need for diagnostic imaging, and orders and interprets reports.
  • Consults with Traditional Healers when indicated and is willing to learn about Traditional approaches to health and wellbeing.
  • Works independently and collaboratively with members of the interdisciplinary team, consulting with physicians and other health care professionals when client’s condition is beyond the RN(EC) scope of practice
  • Performs other related duties as required

Qualifications & Experience:

  • BScN and membership in good standing with the College of Nurses of Ontario, as an RN with Extended Class
  • At least 3 years of experience working as a Nurse Practitioner in the extended role in a community-based setting.
  • Extensive knowledge of clinical best practices for the care and treatment of clients with complex medical and co-morbid conditions.
  • Knowledge and experience with mental health care is an asset but willingness to learn is critical.
  • Excellent communication and relationship building skills
  • Proven ability to encourage contribution and create an environment in which others have the confidence and opportunity to contribute.
  • Commitment to working cooperatively in a community-based setting as part of an interdisciplinary team; ability to collaborate within and across internal and external systems playing a vital role in the delivery of services
