Job Summary:
The Personal Support Worker is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The Personal Support Worker will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The Personal Support Worker will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.
What You Will Do:
Assist clients with physical and mental health needs by fostering respectful relationships, supporting daily living activities, managing medical and behavioral issues, facilitating social engagement, and maintaining accurate care documentation in collaboration with the support team.
Maintain safety and security by responding to emergencies, documenting incidents, ensuring client well-being through regular interaction
Oversee site-specific medical responsibilities by monitoring the medication program, ensuring proper storage and documentation, and collaborating with physicians, pharmacists, and program staff to resolve any issues.
Coordinate and participate in social and recreational activities by attending planning meetings, maintaining participant records, ensuring equipment readiness, communicating client absences, leading outings, including driving clients
Perform various operational and support duties by managing communications, maintaining daily residence operations, coordinating emergency evacuations, participating in meetings and training, working rotating shifts across housing sites, and accepting delegated tasks from senior staff to ensure smooth program delivery.
Other ad-hoc tasks as assigned by the program manager
What You Bring:
A diploma / certificate or above in Human Services, Gerontology or Personal Support Worker from a recognized college or university is preferred
First Aid and CPR training
Experience with clients with unique special needs including cognitive impairment and/or mental health issues.
Knowledge of medication, general health and psychiatric conditions
Ability to organize activities and work with individuals and groups in the areas of assessment, problem solving and crisis management;
Ability to interact with clients in a positive and supportive manner;
Demonstrated ability to work with multi-disciplinary professionals and other staff in a team environment;
Good oral and written English skills.
Computer skills are required
Possession of a valid Ontario Driver's License preferred;
Work Environment:
Physically able to perform demanding body mechanics (e.g. lifting, transferring, etc.)
Since the position involves working in client/client/member’s own homes, staff may be subjected to second hand smoke while performing their job duties.
In addition staff will be required to work rotating shifts as scheduled by the program

This role may follow a Tuesday to Saturday schedule, from 11:00 AM to 7:00 PM.
