Program Summary:
The Specialized Assisted Living - Sherbourne & Respite program delivers individualized support services to people through 8 high-support, permanent residential units in addition to 3 units allocated to the Respite Program. These units alleviate pressure on other healthcare services by offering support for people to transition from hospital settings to the community. Clients accessing both programs may have complex mental health and substance use challenges. The Respite Program offers temporary relief for caregivers or individuals who need intensive support. These short-term, goal-oriented stays provide a consistent space for respite clients to focus on their recovery, which can be related to mental health, substance use, physical health or eviction prevention goals.

The program provides client-centered, individualized services using the principles of Psychosocial rehabilitation. The program offers high-support services and will be staffed with a minimum of 2 staff 24/7. Specialized Assisted Living – Sherbourne and Respite Program operates out of a 3-story site, in downtown Toronto. The property provides single rooms, with shared common spaces (kitchen, washrooms and living spaces). The units on the main floor are accessible, with a ramp to enter the building. Clients can access washroom facilities on the second floor using a stairlift. Respite units will be furnished for clients to allow for a smooth transition to short-term, intensive stays.

Job Summary:
The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support, as assigned, the needs of the clients with mental health and/or addictions challenges. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.

What You Will Do:

  • Client Care & Support: Provide compassionate support for clients’ physical and mental health needs, including medication reminders, health monitoring, and coordination with healthcare professionals.
  • Activities of Daily Living (ADLs): Assist with personal care, hygiene, mobility, meal preparation, and maintaining a clean, safe, and comfortable living environment.
  • Client Engagement & Empowerment: Build respectful relationships that promote dignity, independence, and active participation in daily, social, and recreational activities.
  • Behavioral & Emotional Support: Assist clients in managing behavioural and cognitive challenges, offering guidance, reassurance, and companionship.
  • Safety & Emergency Response: Ensure a safe environment by following health and safety procedures, responding to emergencies, and reporting incidents appropriately.
  • Medication & Clinical Support: Follow established medication protocols, support self-administration programs, and collaborate with healthcare providers to ensure safe practices.
  • Documentation & Team Collaboration: Maintain accurate care records, participate in care planning, and communicate effectively with the interdisciplinary team.
  • Program & Operational Support: Assist with program activities, transportation for client outings, and day-to-day operations, including basic administrative tasks when required.
  • Flexibility & Shift Coverage: Work rotating shifts, provide coverage as needed, and support overall program operations, including during the absence of senior staff.
  • Additional responsibilities may be assigned by the Program Manager and/or designated supervisor to support overall program operations and continuity of care.

What You Bring:

  • Post-secondary education in Human Services or a related field (e.g., PSW Certificate, Gerontology Diploma).
  • Experience supporting seniors and vulnerable populations, including individuals with cognitive and mental health needs.
  • Knowledge of medication administration, health conditions, and psychiatric care.
  • Current First Aid and CPR certification.
  • Experience coordinating social and recreational activities is an asset.
  • Strong skills in assessment, problem-solving, and crisis intervention.
  • Ability to engage clients in a positive, respectful, and supportive manner.
  • Ability to work collaboratively within a multidisciplinary team.
  • Effective verbal and written communication skills.
  • Demonstrated creativity, flexibility, and adaptability.
  • Proficiency with basic computer applications.
  • A valid Ontario Driver’s License is preferred.

Work Environment:

  • Physically able to perform demanding body mechanics (e.g. lifting, transferring, etc.).
  • Since the position involves working in clients’ own homes, staff may be subjected to second hand smoke while performing their job duties.
