Job Summary
As a Psychogeriatric Case Manager, you will be part of a team that co-ordinates a support network and linkages for clients with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect.
LOFT Community Services in partnership with SunnyBrook has an exciting new opportunity for a Psycho Geriatric Case Manager to be part of a community-based health professional team that supports Home and Community Care Residential Accommodation (HCCRA) Programs in the Ontario Health Toronto Region. The goal of the new Centralized Community Health Professions Team (CCHPT) is to facilitate effective transitions to long-term care, supportive housing, home, or a rehabilitation facility. As part of the CCHPT, the PCM will work onsite at different HCCRA locations as part of a multidisciplinary team, that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g. addictions). The PCM will also be supporting treatment plans assigned by OT and/or PT, for clients who are awaiting community transitions, to maintain and progress client’s everyday functioning toward meeting transition targets. A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity and self-respect.
The role will require direct service and support to clients in area such as daily living, practical
assistance, crisis intervention, linkages to primary care and a range of other services to support
the individual’s needs.

Note: Staff will be required to work in clients’ homes and may therefore be exposed to secondhand smoke.
