Schedule: Days with evenings and weekend shifts.

Location: Multi-site: This position will be located primarily at new clinic spaces based in East Mississauga (in development). The position will also occasionally work out of CarePoint Health’s main clinic location at CarePoint Health – 2695 North Sheridan Way, Suite 120, Mississauga, ON

Salary: $66,325-$78,030 and a comprehensive benefits package (including HOOPP pension)

CPH is also the Network Lead Organization for the Ontario Structured Psychotherapy (OSP) Program in Brampton, Halton and Mississauga. OSP offers adults (18+) in Ontario free, short-term, evidence-based cognitive-behavioural therapy and related services for depression, anxiety, and anxiety-related conditions.

What is this role?

We are looking for a Registered Dietitian (RD) who will provide services on nutritional needs based on standards of quality and evidence-based practice. The RD provides consultation and screens patients for nutritional intervention, collects and interprets patient health data for in-depth assessment, completes ongoing monitoring and reassessment in collaboration with patients, caregivers, and the interprofessional healthcare team. Through patient-centered approaches, the RD establishes priorities, strategies and interventions to contribute to the individualized treatment program that allows the patient to achieve optimal outcomes. As an interprofessional team member, the RD fosters a culture of safety by minimizing patient harm, promoting the well-being of patient and health care providers, reducing the likelihood of adverse events, and communicating safety concerns. The Registered Dietician role models professional values, beliefs and attributes, and practices in accordance with CarePoint Health’s mission and values, policies and procedures, and College of Dieticians Standards and Guidelines.

Responsibilities:

Reporting to the Clinical Manager, your key responsibilities would include:

· Providing quality nutrition care in relation to the patient’s diagnosis, treatment, age and nutrition status

· Collaborating with members of the interdisciplinary team to plan, implement, coordinate
and evaluate patient care in consultation with patients and their families

· Providing screening and assessment, contribute to care plan development, establish goal-oriented treatment interventions, patient/family education and provide support to address nutritional needs

· Perform nutritional assessments to children, adolescents, and adults within the context of evidence-based management for chronic diseases such as diabetes, cardiovascular conditions, and GI disorders, palliative care and eating disorders

· Developing the content and material, and conducting part of group sessions related to nutrition

· Working collaboratively with the communications team to develop dietary related health education content

· Using Canada’s Food guide as well as other appropriate resources as a tool for patient care

· Recognizing that clients have conditions beyond their scope of practice and refer to other appropriate health care professionals (e.g. primary care provider, other allied health professionals within the team)

· Accurately takes/records/utilizes the dietician history utilizing the Electronic Medical Record (EMR)

· Designing and implementing a management plan based on presenting conditions/the client’s individual needs

· Participating in the development of CarePoint Health’s programs based on evidence and best practice

· Active involvement in the overall coordination of patient care

· Flexibility to work evenings and weekends as required

· May be required to travel to satellite site or provide home visits

· Other duties as assigned

Qualifications:

· Hold current certificate of registration in good standing with the College of Dietitians of Ontario

· Have successfully completed an undergraduate degree program in a 4-year Dietitians of Canada accredited university food/nutrition program degree/discipline or recognized equivalent

· Have at least 3-5 years of experience providing patient care to patients of all ages in a primary care setting or relevant Ambulatory care setting

· Be experienced working with people living with diabetes &/or complex multi-system chronic diseases and with a mental health diagnosis

· Certified Diabetes Educator Certification (strongly preferred)

· Experience working with pediatrics (strongly preferred)

· Knowledge of disordered eating and best practices in the treatment of eating disorders (strongly preferred)

· Have proficiency in nutritional assessment

· Have previous experience with an electronic medical record (EMR) program

· Demonstrate excellent critical thinking, decision making and problem-solving skills

· Have excellent interpersonal and communication skills

· Demonstrate satisfactory work performance and attendance history

· Competent skill sets in advocating on behalf of clients, interdisciplinary team collaboration, group facilitation support, consultation with professional colleagues and caregivers/families

· Be enthusiastic, passionate and self‐driven who will participate in data management and measurement of quality improvement indicators of patient outcomes;

· Knowledge and proficiency in the provision of crisis intervention, including risk assessment and management form a non-medical perspective, is essential

· Knowledge of community and the resources available, especially in the areas of Crisis services, Mental Health, and Long-Term Care;

· Experienced working with at-risk/vulnerable population

· Be sensitive to patients’ issues and needs and have the capacity to address these issues

· Contribute to program development

· Ensure all communications reflect CPH standards and facilitate a positive patient, family, and provider experience

· Uphold CPH values and be an active contributor to a positive, innovative, collaborative, and cooperative work environment

· Good reading and writing communication skills in English, a second language would be an asset

· BLS certification required

· A satisfactory and current Criminal Background and Vulnerable Sector Screening and proof of immunization required

· Acts as a professional and knowledgeable role model or mentor to colleagues and students; and

· Maintain confidentiality in the workplace and within the community.
