We are currently recruiting for a Social Worker (MSW)

Employment: Temporary Full-time (12-month contract with an opportunity to become permanent)

Schedule: Days with evening shifts.

FTE: 1.0

Location: Multi-site: This position will be located primarily at a new clinic space based in East Mississauga (in development). The position will also occasionally work out of CarePoint Health’s main clinic location at CarePoint Health – 2695 North Sheridan Way, Suite 120, Mississauga, ON

Salary: $66,325-$78,030 and a comprehensive benefits package (including HOOPP pension)

The interprofessional team collaboratively delivers holistic care by addressing patients’ physical health, mental health and social determinants of health.

CPH is also the Network Lead Organization for the Ontario Structured Psychotherapy (OSP) Program in Brampton, Halton and Mississauga. OSP offers adults (18+) in Ontario free, short-term, evidence-based cognitive-behavioural therapy and related services for depression, anxiety, and anxiety-related conditions.

What is this role?

We are looking for a Social Worker who is compassionate and will support individuals, families, groups and communities to enhance their individual and collective wellbeing. The ideal candidate aims to support individuals to develop their skills and their ability to use their own resources and those of the community to resolve problems at hand. This candidate will work collaboratively with team members to provide a continuum of care to patients.

Responsibilities:

Reporting to the Clinical Manager, your key responsibilities would include:

· Providing counselling services, with particular regard to individual and family issues and decisions that impact the health of the patient and family

· Perform client Psychosocial Assessments, Treatment, Psycho-education and Evaluation

· Assess, formulate and provide collaborative care plans with clients.

· Provide short term treatment, outcome evaluation and resource referrals where required.

· Responsible for making clinical decisions and exercising judgement to ensure best possible care is provided within available resources.

· Providing crisis intervention, and brief psychotherapy

· Delivering care in both individual and group settings

· Plan programs of assistance for clients including coordinating appropriate connections to agencies that provide financial assistance, legal aid, housing, medical treatment and other services

· Maintaining communications between the patient, care team and the family

· Working within the primary care model of brief therapy, ensuring seamless transition to community resources as appropriate

· Acts as an advocate for patients and their families in matters affecting health and wellbeing

· Providing house calls and community outreach visits when appropriate

· Collaboration with other CarePoint Health Team members and health care providers in provision of patient care

· Effectively using the Social Work standards of practice and the therapy process in assessing, planning, implementing and evaluating the care of the patient

Qualifications:

· Master’s degree in social work required

· Have clinical experience in a primary health care setting

· Be registered in good standing with the Ontario College of Social Workers and Social Service Workers

· Good working knowledge in the areas of DSM V diagnostic criteria and various modalities of psychotherapy, such as CBT, DBT, motivational interviewing and mindfulness

· Have knowledge and skill in delivering group work including cognitive behaviour therapy, and Stanford self-management programs

· Demonstrate experience with providing sessions virtually and in-person.

· Competent skill sets in advocating on behalf of clients, interdisciplinary team collaboration, group facilitation support, consultation with professional colleagues and caregivers/families

· Be enthusiastic, passionate and self‐driven who will participate in data management and measurement of quality improvement indicators of patient outcomes;

· Knowledge and proficiency in the provision of crisis intervention, including risk assessment and management form a non-medical perspective, is essential

· Knowledge of community and the resources available, especially in the areas of Crisis services, Mental Health, and Long-Term Care;

· Experienced working with at-risk/vulnerable population

· Be sensitive to patients’ issues and needs and have the capacity to address these issues

· Contribute to program development

· Ensure all communications reflect CPH standards and facilitate a positive patient, family, and provider experience

· Uphold CPH values and be an active contributor to a positive, innovative, collaborative, and cooperative work environment

· Good reading and writing communication skills in English, a second language would be an asset

· BLS certification required

· A satisfactory and current Criminal Background and Vulnerable Sector Screening and proof of immunization required

· Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code

· Acts as a professional and knowledgeable role model or mentor to colleagues and students; and

· Maintain confidentiality in the work place and within the community.

· Experience working with children/youth, addictions and trauma patients is an asset
