Job Summary:
As a Case Manager, Psychogeriatric, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. This is a permanent full-time role that will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.

LOFT Back to Home Orillia provides specialized support to vulnerable and at-risk older adults with complex physical health challenges, mental health challenges, addictions or cognitive impairment LOFT responds to each resident’s needs, so they can live safely and in good health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care. Our programs offers 24/7 care through our specialized Personal Support Worker (PSW) team, providing such services as assistance with personal care, essential housekeeping, laundry services, and escorts to appointments. LOFT PSWs receive enhanced training related to mental health, addictions, psychosocial rehabilitation, responsive behaviours, dementia and the impact of the social determinants of health, thus giving them specialized skills required to provide the highest quality of care to the populations we serve.LOFT’s Psychogeriatric Case Managers offer one on-one support and provide care coordination, crisis intervention, system navigation and client advocacy. Our case managers work to connect clients with the services they need to become healthier, happier, and as independent as possible. Our team calls upon a variety of resources, including medical, financial, emotional and recreational supports. Mental, physical and spiritual health are improved by social interaction and the sense of belonging that comes from a welcoming and supportive community. The programs provide medication support, also offer healthy and delicious meals and snacks for residents and is able to accommodate certain special diet requests.

What You Will Do:

  • Assists clients in the development and implementation of a care plan tailored to the client’s needs and issues designed to improve quality of life.  Identify and refer to resources specific to those needs and issues.
  • Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.
  • Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
  • Provide supportive counselling or referral and linkages to health care, programs, or service providers.
  • Provide crisis management/intervention and assist in the development of crisis plans for clients.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles. 
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favorably on the organization.
  • Attend and actively participate in all required training.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners.

What You Bring:

  • Completion of degree in social services/human services or equivalent
  • Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions, and severe social isolation.
  • Group counselling related to mental health, addictions, and related issues.
  • Knowledge of community resources and support.
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities.
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills
  • Good oral and written communication skills and computer literacy, including MS word and Excel.
  • Counselling, responding and assessment skills.
  • Creativity and flexibility to approaches.
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies.
  • Valid drivers’ license, ability to drive work vehicles to transport clients. 

Work Environment:

  • Staff will be required to work in client’s own homes and may therefore be exposed to secondhand smoke.
  • Occasional evening and weekend hours may be required.
  • Frequent travel will be required using own vehicle.
