In this Clinical Manager role, you will leverage your leadership in team building and change management to drive operational excellence and optimize new models of care delivery.

You will lead Multi-Care Kidney Clinics, Home Dialysis, and Kidney Transplant Clinics, ensuring that clinical roles, responsibilities, and workflows evolve to meet emerging healthcare needs. With a strong focus on quality, resource utilization, and professional development, you will empower multidisciplinary teams, foster a culture of continuous improvement, and use metrics to evaluate and enhance program success. DUTIES & RESPONSIBILITIES

As the Clinical Leader of the Ambulatory Kidney Clinics, you will be responsible for:

  • Strategic Planning & Leadership – Develop, implement, and lead short- and long-term plans to support program priorities and organizational goals.
  • Patient Care Excellence – Ensure the provision of high-quality, patient-centered care through evidence-based best practices, continuous quality improvement, and patient engagement initiatives.
  • Inter-Professional Team Management – Recruit, hire, orient, and provide professional leadership to diverse, multi-generational, inter-professional teams. Foster a motivated, safe, collaborative and inclusive work environment. Review and develop models of care to support the clinical needs.
  • Change Management & Innovation – Lead and support staff through organizational and process changes using effective change management strategies.
  • Collaboration with Physicians & Stakeholders – Work closely with physicians, physician leadership, and external partners (e.g., Ontario Renal Network, TGLN) to enhance patient care delivery and service integration.
  • Operational Oversight – Develop, implement, and manage day-to-day policies, systems, and procedures to ensure safe, effective, and efficient clinical operations.
  • Quality & Performance Improvement – Collect, monitor, analyze, disseminate and act on quality performance data, patient/family feedback, and safety reports to drive continuous improvement initiatives.
  • Financial & Resource Management – Plan, manage, and maintain accountability for financial resources, staff schedules, and the physical environment to support safe and effective care delivery.
  • Education & Staff Development – Support professional development through training programs, mentorship, and educational initiatives to enhance clinical and leadership competencies.
  • Regulatory Compliance & Professional Standards – Ensure adherence to professional practice standards, collective agreements, and both hospital and Health Canada policies.
  • Representation & Advocacy – Represent the organization at conferences, workshops, and community/government agencies to support strategic initiatives and advocacy efforts.
  • Corporate & Program-Specific Contributions – Participate in hospital-wide committees, special projects, and initiatives to advance Unity Health Toronto’s vision, values, and goals.

 
QUALIFICATIONS

  • Master’s Degree in Nursing or a related Health Discipline; registration in good standing with a regulatory college.
  • Minimum five (5) years of clinical experience, preferably in nephrology, with expertise in ambulatory care settings.
  • Demonstrated skill in creating professional development plans and fostering a positive learning and inter-professional culture.
  • Proven ability to design, plan, and implement change initiatives, including managing complex clinical and operational projects.
  • Experience in recruitment, staff development and performance management, and resolving HR and labour relations issues (e.g., grievances, investigations, counseling).
  • Demonstrated strong commitment to patient safety, evidence-based practice, and continuous quality improvement to optimize patient outcomes.
  • Demonstrated ability to manage budgets, allocate resources efficiently, and utilize clinical and management data for strategic program planning.
  • Experience in developing, implementing, and evaluating programs/services based on patient needs, service delivery inter-relationships, and organizational goals.
  • Ability to build and maintain strong relationships with internal and external stakeholders to support program and organizational objectives. Previous work with Ontario Renal Network or Trillium Gift of Life an asset.
  • Excellent verbal and written communication, coaching, and team-building skills that promote a collaborative environment.
  • Experience leading projects, facilitating groups, and navigating complex healthcare environments.
  • Dedication to ongoing education and professional development in leadership, healthcare management, and clinical excellence.
