Job Summary: Is a member of the Community Health and the Home and Community Care Team reporting to the Director of Health Services. The Primary Care Nurse Practitioner will provide culturally sensitive primary care services, to MCFN community members across the lifespan and their families, a holistic approach through physical, mental, emotional, and spiritual wellness to chronic disease management, health promotion, disease prevention, end of life support, treatment of disease and injury, and rehabilitation. Working in collaboration and partnership with local Ontario health teams and health service providers and within the MCFN Social and Health teams.

Education and Experience:

• Must have a current registration with the College of Nurses of Ontario as a Nurse Practitioner in good standing.

• Must have a Bachelor of Science in Nursing and a master’s in nursing degree with Extended Class Certificate (Nurse Practitioner) from an accredited university.

• Must have minimum 5+ years of diverse Fulltime RN experience. This experience should encompass working with individuals across the lifespan. Experience working within a First Nations community is an asset.

Knowledge:

• Knowledge of relevant legislation, regulations, and guidelines applicable to programs delivered, e.g., Standards of Practice, College of Nurses of Ontario

• Knowledge of various intervention strategies and evidence-based treatment modalities.

• Knowledge of provincial health legislation, Indigenous health care in Canada, Jordan’s Principle, Non-Insured Health Benefits, Principles for Ethical Research and The First Nations principles of OCAP®

• Solid background in computers with specific knowledge of Microsoft Word, Excel, Power Point Presentation, virtual platforms, and electronic medical records.

Skills/Abilities:

• Ability to work collaboratively with team members and leadership skills.

• Ability to relate effectively to community members, problem solving and critical thinking skills.

• Ability to work effectively with all age groups, strong clinical assessment skills.

• Ability to communicate effectively both verbally and in writing and speak effectively in public.

• Ability to read, interpret and explain policies, programs, reports, financial reporting/budgets.

• Ability to work flexible hours, including evenings, weekends, multi-task, organize multiple projects and programs.

• Ability to write proposals, work plans, and narrative reports.

Other Requirements:

• Must have a valid Class “G” driver’s license.

• Must have a satisfactory result from a Criminal Record Check including a vulnerable sector check.

• CPR and First Aid certification

• Be willing to take the Safe Food Handler’s Certificate

Assets:

• Awareness of the 7 Grandfather’s teachings and the Anishinaabe culture and language.

• Have had Trauma Informed Training.

• Cultural Safety Training and Mental Health training or experience.
