Program Summary:
The Toronto Transitional Age Youth (TAY) Services support youth 16-29 years old from diverse communities. Through empathetic and hopeful relationships, TAY engages and supports the youth, and helps them to build upon strengths towards optimal wellbeing and greater independence. TAY takes a holistic approach, embracing the mind, body and soul to ensure each person receives services in response to their unique needs.
Youth Wellness Hubs Ontario (YWHO) aims to bring integrated low barrier services to youth (12-25) and their families/caregivers. YWHO is improving Ontario’s mental health and addiction landscape for youth and their families by providing rapid access to mental health and substance use services as well as clear referral pathways to interdisciplinary team and community supports.
Job Summary:
The Peer Support Worker supports the collaboration between LOFT and Central Toronto - Youth Wellness Hub ON (YWHO). The Peer Support Worker role will work in collaboration with the partners in the community, including at the YWHO site at 65 Wellesley St E, Toronto, and at the TAY hub, 721 Bloor Street West location. The Peer Support Worker will have a unique role in collaborating with the community and other health and social service providers. The peer support worker shall provide 1:1 peer support and help co-facilitate these groups for youth.
