The Native Canadian Centre of Toronto (NCCT) is a non-profit, charitable organization located
in the heart of downtown Toronto. The NCCT offers a wide range of Indigenous programs and
services based on Indigenous cultural traditions and teachings.
Job Title: Reassurance Worker (Wellness Support)
Employment Term: Full-Time, One-Year Contract, 40 Hours (M-F)
Department: Seniors Department
Direct Report: Seniors Program Manager
Program Overview
The Seniors Program Support Worker provides compassionate, culturally grounded, and
consistent support to Indigenous seniors participating in programming at the Native Canadian
Centre of Toronto. This is a full-time, in-person position focused on wellness check-
ins, reassurance support, and helping seniors remain safely connected to services, community,
and culturally relevant programming.
The role supports the reduction of isolation, promotes dignity, and helps ensure seniors are
aware of and able to access appropriate services and supports.
Key Responsibilities
• Maintain regular contact with isolated seniors/clients to ensure they are not in crisis or at
risk
• Conduct regular telephone contact and in-person wellness check-ins with clients
• Provide reassurance, emotional support, and ongoing engagement to reduce isolation
• Assist clients with completing forms required for services and programs
• Act as a liaison between seniors and external institutions and service providers
• Provide information, referrals, and navigation support to appropriate services
• Assist with the coordination and delivery of seniors programming and events
• Support the coordination of transportation services, including Wheel-Trans
• Participate in the planning and coordination of seniors’ events and activities

• Provide a weekly schedule of client contacts and outcomes to the Case Manager
• Assist in discussions related to long-term care placement when required
• Exercise care in protecting the confidentiality and sensitivity of client information at all
times
• Assist with additional duties as required to support the Seniors Program
Qualifications
• Thorough knowledge and understanding of Indigenous history, culture, and traditional
values
• Experience working with seniors and older adults in a community or support setting
• Ability to work with minimal supervision and manage multiple priorities
• Strong written and verbal communication skills, including report writing
• Familiarity with basic computer applications (email, Word, data entry, scheduling tools)
• Knowledge of the social services sector, including:
▪ Income support programs
▪ Ontario Assistive Devices Program (ADP)
▪ Non-Insured Health Benefits (NIHB)
▪ Income Tax supports and resources
• Strong relationship-building and interpersonal skills
• Ability to manage multiple priorities simultaneously and meet reporting requirements
• Ability to present information clearly and respond to questions from community members
and partner organizations

Working Conditions
• Full-time, in-person position based at the Native Canadian Centre of Toronto

Contract Term
This position is a full-time, one-year contract position. Continuation beyond the contract period
is contingent upon organizational priorities, operational requirements, funding availability,
and program needs.

• Full salary range $45,000.00 - $52,000.00
The successful candidate is expected to contribute positively to the implementation of
the NCCT’s vision and mission, operate in a collaborative team setting and maintain
high and professional productivity levels and quality of service. This is a full-time
position, salary to commensurate with qualifications and experience. Although
preference will be given to Indigenous applicants (First Nations, Metis and Inuit), all
