Job Title: Youth Case Manager

Job Purpose: The Youth Case Manager is responsible for full-time Case Management.
The Youth Case Manager is responsible for providing direct support to clients by using harm
reduction, client-centred, anti-oppressive and trauma-informed approaches. The Youth Case
Manager will carry a caseload of clients who are experiencing a range of challenges such as,
but not limited to homelessness, living in poverty/unsafe environments, living with mental health,
traumas, substance use, unemployment, social isolation, and/or history with the criminal justice
system.
Duties and Responsibilities:
● Provide one-on-one case management and advocacy for Indigenous youth, ensuring a
strengths-based, culturally grounded approach.
● Develop individualized case plans in collaboration with youth, outlining short- and
long-term goals.
● Promoting awareness and participation in centre activities
● Working in a manner that preserves confidentiality and seeks to minimize risk
● Build trusting relationships with youth, families (where appropriate), Elders, and
community members
● Maintain accurate, confidential case notes and documentation using NCCT database
systems
● Responds to enquiries, complaints or emergency situations involving clients
● Collaborate with internal and external service providers to ensure coordinated care
● Utilize community resources effectively to maximize the client's independence and to
enhance the client's quality of life
● Works cooperatively and maintain communications with team members and other
service providers to provide a continuum of services and supports to youth
● Coordinates services and follow-ups with youth and agencies to document
use/success of referrals
● Documents youth-related activities accurately and in a timely manner within a
computerized case management database
● Creates and maintains record keeping for all client related matters and prepares
correspondence and statistical information as per policy and required by funding
agreements
● Supporting the centre’s student and volunteer placement programs
● Coordinates skill-building activities and engages residents as required by funding
agreements
● Maintains current knowledge of legislation, services, resources, policies, programs,
procedures and issues affecting clients
● Participates in regular supervision, case reviews, team meetings, youth council
meetings and training sessions
● Commits to learn and use an Indigenous language reflective of NCCT community
● Other related duties as may be assigned from time to time
QUALIFICATIONS:
● Bachelor’s degree or Diploma in Social Work or related field
● Minimum of two years related experience providing support to vulnerable youth living
with or at risk of mental illnesses, problematic substance use and/or addictions
● Knowledge of cultural sensitivity to Indigenous communities and cultures
● Experience working in an on-site housing setting and providing services in this
environment.
● An understanding of the therapeutic relationship, and an ability to agree with clients on
goals/priorities while respecting their autonomy and individuality
● Ability to practice active listening skills and ability to interact with clients in a positive
and supportive manner
● Crisis intervention skills, with well-developed problem-solving abilities
● Strong knowledge of community services and other relevant agencies
● Strong computer skills and ability to learn new programs relatively quickly
● Excellent oral and written communication skills
● Ability to work effectively within a team environment and to interact effectively with
groups
● Current Vulnerable Sector Screening Check
WORKING CONDITIONS:
● This position requires a person to be able to interact with youth who may have
complex needs such as mental health, addictions, developmental disabilities and so
forth. Occasional evenings and weekends as required.

● Full salary range ($60,000 - $65,000) Hiring salary $60,000

Salary is commensurate with qualifications and experience. Although preference will be
